Collaboration brings disability support plans to life

No one who works in the disability sector would ever say that Support Coordination was simple. It’s complicated. Which of course is why it exists, to help people living disability navigate their NDIS disability support plans and understand the support options available.

Every now and then bright moments that shine through in the world of disability services and support organisations. Like when Elaine, one of Leisure Networks incredible support coordinators, received a selection of photos of one of our NDIS support coordination customers, Michael, who was out in public enjoying the autumn sunshine.

This might seem like a mundane and everyday event form most of us, but for Michael it was groundbreaking!

Michael lives with complex psychosocial and intellectual disability, further impacted by a limited grasp of English, which he speaks as a second language. Elaine has been working with Michael and his support network for almost a year now bringing together the supports he’s needed to overcome many challenges, including sitting in a car and being in a public space.

After many months of careful care planning, this was truly a day to celebrate!

What a team effort! Thanks to Residential Team Leader, Anne, from Gateways Support Services Inc.; Positive Behaviour Support Practitioner, Amy, from Outcomes Connect; Occupational Therapist, Chloe, from OT Essentials; Speech Therapist, Thien, from Allied Therapy Services, and Michael’s sister Sandra.

When you reflect on the sheer number of people and supports needed, you do get a clearer appreciation for what an amazing day this was!
