Specialist Support Coordination

Helps people address the complexity that affects their access to supports, enabling them to overcome any significant barriers in plan implementation. Our experienced Coordinators work with NDIS participants whose situations are more complex and require a higher level of support to manage the challenges in their support environment.

We are here to help build your capacity and confidence. We support you in making sense of the NDIS and help you to gain the confidence you need to action your plan as well as managing the priorities in your life.
The team will connect you with new community activities, explore new service providers and assist you to achieve your desired outcome.

  • Exploring new services to support you in your everyday life.
  • Developing meaningful and fulfilling relationships
  • Exploring different housing options

  • Monitoring your funding, supports and progress towards your goals.
  • Connecting you with the important things within your community.
