Working in the Community Connections team, the Support Coordinator role provides varying levels of coordination and transition supports for children, young people and adults.  The primary focus of this role is to enable an individual to build their capacity with the aim of independently managing their issues and supports whilst meeting their goals and exercising choice in obtaining supports that assist them in pursuing their own lifestyles.  Please view the Position Description for further information about this role;

Disability/Inclusion Support Workers - Wyndham (Casual)

As Leisure Networks continue to expand our programs and activities for kids, teens and adults, we are seeking experienced Disability Support Workers to help provide support to deliver these programs, in order to enable our participants to transition to a desired level of independence. Based in Geelong and/or Wyndham, we have multiple positions to offer on a casual basis, with shifts including weekdays, evenings and weekends. We are looking for enthusiastic and creative people who are passionate about engaging others and promoting social and community inclusion.  Previous disability experience is essential.
