Disability Support Service

General Statement of Purpose

To provide flexible and equitable support services to people with disabilities which assist them to integrate into the wider community, expand their choices, and optimise their overall health and wellbeing.

Target Group

People with disabilities who are able to communicate their own aspirations/goals.

Objectives

  • To work collaboratively with all key stakeholders
  • To provide person centered planning and support to individuals
  • To identify and address barriers to achieving community integration
  • To keep abreast to current policies, services, and trends
  • To provide a flexible and relevant services
  • To advocate for the needs of people with disabilities
  • To develop, deliver and evaluate relevant activities and/or programs, in consultation with all stakeholders  
  • To provide relevant support and education to the community 

The Disability Support Service Team provides individual support services to clients funded through a range of State Government programs, such as Futures For Young Adults, and Individual Support Package .  The individual support provided aims to meet the individual client’s specific needs and enables them to make informed choices regarding their lives.

To qualify for Leisure Networks’ Individual Support Service a person must first go through the Department of Human Services Disability Intake for assessment.

Persons with a real desire to be included in the broader community are fully encouraged to access Leisure Networks’ Individual Support Service. Initial contact with the service is with the Client Services Coordinator. A person centered individual plan is then developed for each client based on their stated goals. Plans are further broken down to reflect how these goals will be achieved, including how, when and where to access relevant organisations and/or individuals. The plan can be creative and flexible and can focus on anything from, education and employment, to transport options and solutions, to social and leisure pursuits. A key worker is then assigned to the person to provide the client with ongoing support in monitoring, achieving and evaluating their goals, and assisting meet any other needs that arise. The person will be encouraged to fully explore their interests and potential through accessing opportunities in the community that are available to all.

Leisure Networks has extensive networks with the sport and recreation; education and employment; and disability and youth sectors, as well as the broader community. Through these networks clients can be linked with organisations and individuals providing services and activities that assist meet the goals of their individual plans, and in turn, increase general individual fulfillment.

All Abilities Sport

Leisure networks through its Access for All Abilities program supports sport and recreation clubs in the Barwon region to implement inclusive programs for people with disabilites.
